Convert to Care Plan – Warm Referrals

Converting to a care plan after an evaluation should come off as the “next step” to the client as long as you’ve been positioning that this is how you work. If the evaluation has shown room for improvement, the care plan is that affordable and efficient way to achieve improvement while doubling as an ongoing service that protects their investment (their website). Since the client you likely referred them to you in on a care plan, you can refer back to that relationship and how the care plan has helped them.
